RE: DEPARTMENT QUESTIONNAIRE ON Y2K COMPLIANCE

The California Department of Insurance has requested that the SLA distribute the enclosed 7-page questionnaire (479K) (PDF Format) to each Surplus Line Broker licensee.  Enclosed is a letter from the Deputy Commissioner, Enforcement Branch, Mark A. Lowder explaining that the questionnaire is intended to determine if your brokerage operation is Year 2000 compliant.  The Department considers this a very important consumer protection issue and requires that each licensee complete and return this questionnaire by February 12, 1999 to the following address:

Failure to complete and return the questionnaire will result in further action by the Department and may jeopardize your license to do business in California.
